How TXA Stops Bleeding

Tranexamic acid — mechanism of action, from fibrin formation to fibrinolysis

Common misconception: TXA does not add, activate, or boost clotting factors. It has zero direct procoagulant effect — it works entirely by blocking the breakdown of clots that have already formed. It's an antifibrinolytic, not a clotting agent.

Key Concepts
The Mechanism
Lysine-Binding Site Blockade
TXA is structurally similar to lysine. It reversibly occupies the lysine-binding sites (kringle domains) on both plasminogen and plasmin, blocking attachment to fibrin.
The Key Enzyme
tPA's Role
Tissue plasminogen activator is released by injured vessel walls near a clot. It converts plasminogen → plasmin, working mainly once both are already fibrin-bound.
What It Is NOT
Not a Procoagulant
TXA doesn't raise platelet activity, add clotting factors (unlike FFP/cryoprecipitate), or change the coagulation cascade. Only the breakdown side is affected.
Why Timing Matters
The 3-Hour Window
TXA only works on clots that already exist. CRASH-2 showed a mortality benefit within 3 hours of injury — after that, evidence suggests potential harm.
The Bigger Picture
COAGULATION FIBRINOLYSIS

These two systems normally stay balanced — a clot forms, does its job, then dissolves over days as the vessel heals. In major trauma, this can tip toward hyperfibrinolysis — clots the body needs to survive get broken down too fast. That imbalance is exactly what TXA is treating.
